MCHC stands for mean corpuscular haemoglobin concentration. It measures how densely packed with haemoglobin your red blood cells are, and it's one of several red cell indices reported in a full blood count. A mildly abnormal MCHC on its own is common, is usually calculated rather than directly measured, and rarely means something serious by itself.
MCHC is one of those results that causes disproportionate worry, mostly because it's an unfamiliar abbreviation sitting on a page full of other unfamiliar abbreviations. In practice, it's rarely the number that changes what we do next. It's what it's doing alongside MCV, MCH and ferritin that actually matters, and I've had patients fixate on a single borderline MCHC while barely glancing at a ferritin result that told a much clearer story.
What is MCHC and what does it actually measure?
MCHC calculates the average concentration of haemoglobin inside your red blood cells, reported in grams per litre. The NHS reference range is typically 310 to 350 g/L, though this can vary slightly between laboratories.
It's calculated from two other full blood count results, your haemoglobin and your haematocrit, rather than measured directly. That's worth knowing, because it means MCHC reflects the relationship between those two numbers as much as anything measured on its own.

What does a low MCHC mean?
Low MCHC most commonly points toward iron deficiency, where red blood cells are produced with less haemoglobin packed into them than usual, sometimes described as hypochromia. A single mildly low MCHC is one of the most frequent minor findings on a full blood count.
This could suggest it's worth checking ferritin and iron studies alongside a low MCHC, particularly if you're also experiencing tiredness, though a mildly low result in isolation often isn't clinically significant.
What does a high MCHC mean?
High MCHC is less common than low MCHC and, above roughly 36 g/dL, is often a laboratory artefact rather than a true biological finding, caused by things like cold agglutinins or a lipaemic sample interfering with the analyser.
When a genuinely high MCHC does reflect biology, it's more often linked to hereditary spherocytosis or severe autoimmune haemolytic anaemia, both of which are usually picked up alongside other clues in the full blood count, not from MCHC in isolation.

MCHC, MCH and MCV: what's the difference, and why look at all three?
MCV measures the average size of your red blood cells. MCH measures the average amount of haemoglobin in each cell. MCHC measures how concentrated that haemoglobin is relative to the cell's size.
A low MCV alongside a low MCHC is the classic pattern seen in iron deficiency anaemia: small, pale cells with less haemoglobin packed in relative to their size. A normal MCV with a borderline MCHC is a different, less specific picture, and usually prompts a second look rather than an immediate conclusion.
What are the limitations of MCHC on its own?
Because MCHC is an average across all your red blood cells, it can look completely normal even when you have two distinct populations of cells behaving very differently, for example if you have both iron deficiency and a B12 or folate deficiency at the same time.
This is exactly why MCHC is never interpreted alone. A full blood count, ideally alongside iron studies, B12, folate and, where relevant, a blood film, gives a far more complete picture than any single red cell index.
Myth versus fact: a normal MCHC rules out anaemia
Myth: If my MCHC is normal, I can't be anaemic.
Fact: MCHC is an average, and anaemia is actually defined by a low haemoglobin level, not by MCHC. It's entirely possible to be anaemic with a completely normal MCHC, MCV and MCH, a pattern called normocytic normochromic anaemia, which is common in chronic disease or after acute blood loss.

Full blood counts also report RDW, or red cell distribution width, which measures how much your red blood cell size actually varies, rather than just the average. A high RDW alongside normal-looking average indices can be an early clue that two different processes, such as iron deficiency and B12 deficiency together, are affecting your red cells in different directions at once, something the averages alone can hide.
What to know before you book a full blood count
No fasting is required for a standard full blood count, including MCHC, MCV and MCH.
If you've recently had a significant infection, illness or blood loss, mention this when booking, since it can temporarily affect several full blood count results.
If you're already being investigated for anaemia, it's often more useful to test iron studies, ferritin, B12 and folate at the same time as your full blood count, rather than the full blood count in isolation.
Menstruating women with heavy periods, and anyone following a vegan or vegetarian diet, may want to specifically request iron and B12 markers alongside their full blood count given the higher likelihood of relevant deficiencies.
What happens after your full blood count results come back?
A normal full blood count, including MCHC within range, generally needs no further action unless you have ongoing symptoms that prompted testing in the first place.
A mildly low MCHC alongside otherwise normal results is often followed up with ferritin and iron studies, to check whether early iron deficiency is developing before it affects your haemoglobin significantly.
A more significantly abnormal result, particularly alongside symptoms like persistent tiredness, breathlessness or unusual bruising, is worth discussing with a healthcare professional rather than waiting for your next routine check.
The best way to interpret MCHC is never in isolation. It's read alongside MCV, MCH, RDW, the rest of the full blood count, and, if there's any suggestion of anaemia, your ferritin and iron studies too.
